documents/File/EMS Instructor Tracking Form 11-12-08 revision.pdf  NEW!**This form is to be utilized to 'track' the pre-requisite 20 hours of assistant instructing time prior to taking an EMS Instructor course. This form will also be used to track the required 12 hours of supervised instructing time after successful completion of the EMS Instructor Course.

EMS Instructor certification is linked with your Practitioner card; this means the expiration of both certificates is the same.  The exception is ALS Practitioners as your paramedic or PHRN card has no expiration date, yet your EMS Instructor certification does expire every three years.  It is the responsibility of each instructor to verify that they have met the requirements for renewal.
